The OUTSTANDING MENTOR AWARD will be presented to the female coach, fitness instructor, or recreation specialist who best exemplifies the characteristics of a coach, teacher, and mentor of female athletes of all ages. Nominee should have made significant contributions to her sport community. Nominee can include any high school, college, amateur sport club or youth sport female coach who is not a national, professional, or an NCAA Division I or II coach. We wish to recognize those women who work at the "grassroots" level.
Judging Criteria:
Please address the following judging criteria on a separate sheet of paper and explain how the nominee meets these attributes and why she is deserving of this award. Embraces a positive attitude that is an inspiration for others.
- Encourages and supports others to enjoy sports.
- embodies the true spirit of sports and fitness.
- Contributes to the sports community.
- Advocate of girl's and women's sports.

How to Nominate a Candidate:
- Complete nomination information below, then click "Submit Application" button.
- Mail a testimonial letter addressing the judging criteria.
- Include 3 letters of recommendation and other supporting material (newspaper clips, brochures, programs, etc.)
- 5 X 7 black and white photo to be used in program.
